Allen County Health Department-  $47,752

  • The goal of the “Women’s Preventive Healthcare Project is to alleviate the threat of breast cancer in women and men.  The project provides screening mammograms for residents who reside in Allen, Auglaize, Hancock, Hardin, Mercer, Putnam, and Van Wert counties who are uninsured and 200% of federal income guidelines. 419-224-4500 or 1-800-901-9727 http://www.allencountyhealthdepartment.org/breast_cervical_cancer.htm

 

Blanchard Valley Health System- $28,800

  • The goal of the “Hancock County Mammography Screening Initiative” is to provide screening and diagnostic mammography service to uninsured and underinsured men and women who reside in Hancock County.  The project will provide mammograms, ultrasounds, biopsies , ductograms, and MRIs to residents that are at-risk for breast cancer. 419-425-3170 http://www.bvhealthsystem.org/?id=651&sid=1

 

Fulton County Health Department- $85,000

  • The goal of “A Woman First” project is to lower breast cancer mortality in Northwest Ohio through early breast cancer detection.  The project provides breast health education, screening mammograms, diagnostic services (i.e. mammograms, ultrasounds, biopsies), and interpreter services.  The project services low income, uninsured residents of Defiance, Erie, Fulton, Henry, Lucas, Ottawa, Paulding, Sandusky, Wood, and Williams counties in Ohio.  419-335-3907 or 1-800-929-6626 http://www.fultoncountyhealthdept.com/OurServices/Nursing/BreastandCervicalCancerProject.aspx

 

Hospital Council of Northwest Ohio- $104,461.00

  • The goal of the “YES MAMM!” project is to increase awareness and utilization of breast cancer prevention, screening, and treatment resources available through six partner hospitals within 7 Ohio counties in the Komen Northwest Ohio Affiliate.  Each of the six hospitals will facilitate one or two day screening events for uninsured patients in their service area.  The partnering hospitals are Henry County Hospital (Henry County), Fisher Titus Medical Center (Huron and Erie counties), Magruder Hospital (Ottawa and Erie counties), Paulding County Hospital (Paulding County), Memorial Hospital (Sandusky County), and Wood County Hospital (Wood County).  The project will impact the 7-county region by increasing mammography screening rates and increasing breast health knowledge.  419-842-0800

 

Mercy Memorial Hospital- $28,000

  • The goal of the “Put Yourself First Today!” is to increase access to breast cancer screening and diagnostic services in Monroe County, Michigan.  The project provides screening and diagnostic mammograms, ultrasounds and MRI to uninsured and underinsured residents of Monroe County, Michigan.  734-240-4200 http://www.mercymemorial.org/serv_women.php

 

Mercy Tiffin Hospital- $19,686

  • The goal of the “Mammogram Assistance” project is to increase access to breast cancer screenings and lower breast cancer mortality.  The project provides mammograms to low income and uninsured residents of Huron, Seneca and Wyandot counties.  419-455-7435

 

Mercy Women’s Care at St. Anne’s and St. Charles- $50,750

  • The goal of the “Mammography Assistance Program (MAP)” is to provide comprehensive breast cancer education and screening for low income residents of Monroe County, Michigan and Lucas, Ottawa, and Wood counties in Ohio.  Outreach and education is implemented by reaching out to residents in churches, homeless shelter, and community centers.   Mammogram screening services are provided at three Mercy Health Partner Hospitals: St. Anne, St. Charles, and St. Vincent.  419-696-7314

 

North Coast Cancer Foundation- $20,000

 

ProMedica Cancer Institute- $40,000

  • The goal of the C.A.R.E.S. (Collaboration, Access, Reduction of barriers, Empowerment and Screening) project is to ensure that women and men have access to recommended clinical breast exams, screening mammograms, education and navigational support for breast cancer prevention and early detection.   The project will work with Bay Park Community Hospital, Bay Women’s Services Port Clinton, and Fostoria Community Hospital.  The project provides services to men and women in Hancock, Lucas, Ottawa, Seneca, and Wood counties.  419-824-1126 http://www.promedica.org/default.aspx?PageID=1650

 

Putnam County Health Department- $12,256

  • The goal of the “Putnam County Breast Health Project” is to increase access to breast cancer screenings in Putnam County to identify breast cancer early and therefore lower the mortality rate.  Men and women that are uninsured or underinsured will have access to mammograms, breast ultrasounds, and biopsies. St. Luke’s Hospital- $29,284

  • The goal of the “Mammograms for Men and Women” project is to decrease the death rates from breast cancer through early detection and treatment.   The project provides clinical breast exam clinics and mammograms for uninsured men and women in Fulton, Henry, Lucas, Ottawa, and Wood counties.   419-893-5988

 

Toledo-Lucas County CareNet- $37,500

  • The goal of the “CareNet- Helping to Keep the Komen Promise” is to reduce barriers to attaining/maintain breast health for low income residents of Lucas County.  The project assists low income, uninsured residents receive mammograms, breast cancer treatment and breast health educational materials while reducing barriers such as cost and transportation.  Partners of the project include ProMedica Health System, Mercy Health Partners, University of Toledo Medical Center, St. Luke’s Hospital, The Pharmacy Counter, Toledo Area Transit Authority, and volunteer surgeons and oncologists.  419-842-0800

 

The Victory Center- $18,000

  • The goal of the “Discovering Inner Strength: Support for Women with Breast Cancer”  is that women living with breast cancer will experience a higher level of coping with their diagnosis through participation in free, safe, and effective complementary therapies.   The project conducts a monthly Breast Cancer Survivor Support Group and a Breast Cancer Caregiver Support Group.  The project provides breast cancer patients and survivors with services that support the mind and body such as massage, reflexology, reiki, healing touch, meditation, facials, counseling, yoga and Healthy Steps exercise.  419-531-7600

 

Wood County Health Department- $13,785

  • The purpose of the “Wood County Women’s Breast Health project is to provide breast cancer preventative and educational services to under and uninsured women in Wood County, Ohio.  The project will provide clinical breast exams through the Wood County Health Department, and screening and diagnostic mammograms at the Wood County Hospital.   The project will decrease breast cancer mortality rates in the county through decreasing barriers and increasing access to care, education and early detection for under and uninsured women.   419-352-8402

 

Wyandot County General Health District- $13,200

  • The goal of the “Wyandot County Breast Health Initiative” is to provide access to breast health education and breast cancer screening.  The project will collaborate with Wyandot Memorial Hospital, private medical provides, and other community partners to identify low to middle income residents in need of a mammogram, breast ultrasound, and referral services.   The project provides breast cancer screening and diagnostic mammograms, breast ultrasounds, transportation assistance and care coordination for residents in Wyandot County. 419-294-3852 http://www.wyandothealth.com/news/articles/000002/000234.htm

 

 Komen Northwest Ohio 2011-2012 Small Grants

 

Grantee Organization: Bowling Green State University

Program Title:   C.U.R.E. (Community United through Relationships and Exercise)

Amount: $6,000

Counties serviced: Wood County

Summary:   The goal of “C.U.R.E.”  is to provide breast cancer patients and survivors a safe exercise program and support system that encourages healthy lifestyles to prevent breast cancer recurrence.  The program components include water exercise, strength training and aerobic exercise taught by instructors trained in “Cancer and Exercise” by the American Council on Exercise.

 

Grantee Organization: First Call for Help, Inc.

Program Title:  Breast Cancer 2-1-1

Amount: $8,000

Counties serviced:  Defiance, Fulton, Henry and Williams counties

Summary:  The  goal of the “Breast Cancer 2-1-1” program is to connect breast cancer survivors with needed recourse during their recovery through a  comprehensive breast cancer resource database.  The program will also develop a network of breast cancer service providers that will meet three times a year to ensure information in the breast cancer database is the most current. 

 

Grantee Organization: Health Partners of Western Ohio

Program Title:  You are Precious!

Amount: $5,000

Counties serviced:  Allen County

Summary:   The goal of the “You are Precious!” program is to educate 800 African American women in Allen County about mammography, the importance of screening and early detection of breast cancer, and the availability of financial assistance programs through evidenced-based strategies for promoting breast cancer screening among underserved women.

 

Grantee Organization: Joint Township District Memorial Hospital

Program Title:  30th Annual Breast Imaging Conference Training Grant

Amount: $1,840

Counties serviced: Allen, Auglaize, Mercer, and Shelby counties

Summary:   The small grant will send a Women’s Imaging Center staff to attend the 30th Annual Breast Imaging Conference to receive reinforced and advanced training and education in the areas of breast health and diagnostic breast cancer.  The Women’s Imaging Center staff will then share the information with other health care professionals.

 

Grantee Organization: Joint Township District Memorial Hospital

Program Title:  Take Time for You! In Celebration of Breast Cancer Awareness Month

Amount: $3,000

Counties serviced: Allen, Auglaize, Mercer, and Shelby counties

Summary:   The goal of the “Take Time for You!” program is to provide breast health education and provide assistance with scheduling a mammogram for  125 at-risk women in the targeted counties with a focus on those that are 45 years and older.

 

Grantee Organization: Seneca County General Health District

Program Title:  Breast Health Education of Seneca County

Amount: $6,000

Counties serviced: Seneca County

Summary:   The goal of the “Breast Health Education of Seneca County” is to provide 21 breast health education workshops to three at-risk populations in Seneca County: (1) high school juniors and seniors, (2) teenage girls that are in the detention center or on probation, and (3) incarcerated women. 

 

Grantee Organization: University of Toledo- Health Science Campus

Program Title:  From Gray to Pink: Helping Older Adults with Breast Cancer Understand and utilize the Health Care System in the Real World.

Amount: $2,295

Counties serviced: Fulton and Lucas counties

Summary:   The goal of the “From Gray to Pink” program is to improve the quality of life and decrease anxiety in older patients with breast cancer through a session of workshops in collaboration with local senior citizen centers.  The workshops will increase self-confidence, efficacy, and communication with health care providers to create a sustainable population to continue to educate and support survivors for generations.
