How to Apply for Funding

Community Grants

Due October 14, 2011

Awarded for innovative programs that reduce breast cancer incidence and mortality rates, especially those who are disproportionately affected by breast cancer. All programs must address breast health and/or breast cancer education, screening, diagnosis, treatment, or survivorship needs of individuals in the Komen Northwest Ohio Affiliate’s 24-county service area. Community Grant applications are accepted once a year and should address at least one of the four priorities identified by the Affiliate:

  • Breast Health and Breast Cancer Education/Awareness: Programs that focus on providing culturally appropriate breast cancer education and awareness, patient navigation, and advocacy with an emphasis on the medically underserved.
  • Access to Breast Cancer Screening: Screening of high-risk underserved populations, especially focusing on women and minority populations of all ages, with resource and access barriers.
  • Access to Breast Cancer Diagnosis and/or Treatment: Diagnosis and treatment of high-risk underserved populations, especially focusing on women and minority populations of all ages, with resource and access barriers.
  • Breast Cancer Survivorship: Programs directed to breast cancer patients at the early or newly diagnosed stage and ongoing through treatment.

 


Small Grants

Awarded in amounts up to $10,000 to support breast health and breast cancer education/service projects, conferences, and travel scholarships.  Due to the high volume and quality of grant requests received, the Northwest Ohio Affiliate of Susan G. Komen for the Cure has distributed the budgeted amount of $45,000 for small grant requests in the first 2-cycles (May and September) of the current fiscal year.  As a result, the January 15, 2012 small grant application deadline has been closed.  The next small grant application deadline will be July 15, 2012.

The Northwest Ohio Affiliate’s Grant Program is dedicated to providing equal access to quality breast health care for all. The Northwest Ohio Affiliate service area encompasses 24 counties including Monroe County in Michigan and the following counties in Ohio: Allen, Auglaize, Crawford, Defiance, Erie, Fulton, Hancock, Huron, Hardin, Henry, Logan, Lucas, Mercer, Ottawa, Paulding, Putnam, Sandusky, Seneca, Shelby, Van Wert, Williams, Wood, and Wyandot.


Seventy-five percent of net funds raised by the Affiliate remains in the Northwest Ohio Affiliate's service area to support community-based breast health programs focusing on the medically underserved and uninsured populations.  Since its inception in 1999, the Northwest Ohio Affiliate has awarded over $7 million in grants. The remaining twenty-five percent is contributed to the Susan G. Komen for the Cure® National Research Grants Program.
